Please put the rejected date.
If less than two years, the appointment time will be longer and maybe on specific day.

现在是:passed, failed, checked 能不能改成: passed, failed, checked, check and passed, check and failed.
或者索性分成两项: 结果:passed, failed 有否check: yes, No
