For people having problem for visa and might made mistakes during the 1st or 2nd application, check Hong Kong US consulant
My wife has problems in Chengdu, Beijing. But HK officer are very nice. I heard from them that 90+% non-immigration visa got granted.
Try to apply HK visit passport, and then you could go to the consulant any time, no reservation needed. To go back, take bus from HK to Shenzhen airport (2hrs), that will save you some airfare.

I was told that it's the location where you lived for 6 months before you went to U.S.
But I still went to Chengdu. They did not question this.
