DS160问题：三个字的名字，曾用名需要选Yes? 名字的电报码需要写吗？

看不到例子，猜测应该这样，比如“张老三”： Zhang Lao San。对吗？

需要在确认页上写上名字的电报码？以及名字、家庭地址、公司名字和地址的中文吗？

不知这个页面上要求的这几条现在是不是还需要，多谢指点。
