You only fill in the visa you got stamped on your passport. When you change your F1 to H1 in US, you only changed your immigration status, you were not issued a visa. So if you have never been back to China and never got stamped a H1b visa, you only fill in your F1 visas.
PERM is not immigration visa application. So fill NO.
