网上有两种DS157&DS158.一种是doc文件，一种是pdf文件：http://shanghai-ch.usembassy-china.org.cn/forms.html
应该填哪一种呢？doc or pdf? or both?
要用手填？还是在电脑上填再打印？

either doc or pdf will work. as long as it is from embassy's website
either fill in on computer, then print or fill with hand is OK. as long as the information are readable and accurate.
